Field Trip Permission Form

Wetlands Edge Environmental Center (WEEC) is located on BP's Wildlife Habitat and with access to 3M Decatur Wildlife Habitat, 3M Decatur's Wildlife Habitat, or Wheeler Wildlife Refuge, natural settings that include wetlands, gravel trails, and areas which are both forested and grassy. As natural habitat areas, there is always the possibility of wildlife encounters that may include insects, spiders, ticks, and snakes. Vegetation hazards such as poison ivy and vines with stickers may also be present. There also may be natural slipping or tripping hazards in the habitats.

When your child visits WEEC and the BP Wildlife Habitat or 3M Decatur Wildlife Habitat or Wheeler Wildlife Refuge, s/he will be under the direct supervision of his/her classroom teacher(s), chaperones, and WEEC staff members. Every effort has been and will continue to be made to ensure your child's safety while on the field trip. In turn, your child is expected to adhere to Decatur City Schools' Code of Conduct and WEEC rules. If you feel that your child will need special accommodations to visit the center and the habitat, please provide the appropriate information below.

General questions regarding the field trip may be answered by referring to the WEEC Field Trip information sheet in the packet provided by your child's teacher. Questions regarding special accommodations may be referred to Dr. Stefanie Underwood, Special Services Supervisor, at 552-4637 or WEEC staff members at 301-1559.
