Required  K-12 Permission Forms

Students from Decatur City Schools must have Forms 1 and 2 signed by their parent/guardian in order to visit WEEC. Out-of-District public school students need only Form 2 signed. Please download and print the forms for distribution to your students.  On the day of your field trip, please bring Forms 1-3 (DCS) or Forms 2-3 (Out-of-District public schools) with you.

1. English or Spanish  DCS/WEEC Permission Form

2. English or Spanish  bp Liability Release Form
3. Checklist / Class Roster to list students, teachers, and chaperones who will participate in the field trip, plus any pertinent information such as if special accommdations are needed.

The following forms may be of use to you.  Feel free to use them if needed.

4. Field Trip Reminders for DCS Teachers or Out-of-District Teachers including student behavior expectations and safety information.

5. English or Spanish Parent Information  (This is a sample formthat may be added to for your particular class.)
